Dear Property Owner:
The City of Slidell pursues every opportunity to mitigate properties in our community. Currently, we are preparing an application through the FMA (Federal Mitigation Assistance) grant program. This is a competitive program among applicants from across the country, and there is limited funding available. If the Slidell application is funded, we will be able to offer eligible Severe Repetitive Loss (SRL) applicants as much as 100% of your eligible mitigation costs.
Attached are the forms that we need completed if you wish to participate in this grant:
- Homeowner Application - Make sure to include all attachments listed on the application!
- Assignment of Coverage Form
- W-9 Form
- Bid Form – Must obtain 2 bids on same Scope of Work! This means that both contractors should bid on same elevation height and same number of stairs and landing configurations.
- Duplication of Benefits Affidavit - Must be notarized!
- Acknowledgement of Conditions – Must be notarized!
- Voluntary Participation Agreement - Must be notarized!
In addition to the attached forms, we will also need you to provide the following:
- Current flood insurance (must remain current)
- Elevation Certificate or
- Colored photos- front, back, and each side of the home that is to be elevated
There is no guarantee that your project will be included in this year’s application. It will depend on your project costs, number of insured losses, and other inputs into the FEMA Benefit Cost Analysis (BCA). All interested applications will be ranked by BCA and the City will include as many applicants as possible and stay within the limitations of the grant priorities. We will mail you a letter after the application is submitted to let you know if your application was included or not.
